Case Management

Assisting the  uninsured client obtain a meaningful level of needed health related support services is a goal of the Hinds County Health Alliance. This presents a myriad of challenges as well as opportunities. The Alliance has a full time case manager who provides a wide range of services which include:   

 Assessment for CHIP/Medicaid eligibility
 Securing primary care provider
 Prescription drug assistance
 Transportation assistance
 Coordinating social service needs
