The Next level (Level 3) of the network is the Community Hospital. This is where patients in need of hospitilzation will
be admitted and treated. However, in this model, although hospitalized, the patient remains a responsibility of the entire
network, meaning all the resources of the health house and community clinic remains available to the patient.
When patient leaves the hospital the health house community health worker is there to assure compliance with discharge
instructions, thus reducing the need for re-admission for the same diagnoses.
